We may use any Personal Information listed above as we believe necessary to:

  1. Enforce our rights or agreements if a policy violation has occurred;

4. Your Choices Regarding Your Personal Information

A. Your U.S. State Privacy Rights (Virginia VCDPA & Maryland)

Depending on your state of residence, you may have rights with respect to Personal Information we hold about you. For Virginia residents, these rights are provided under the Virginia Consumer Data Protection Act (VCDPA). Maryland residents may also have rights under applicable Maryland privacy laws. To the extent provided by applicable law, you may have the right to:

  • Access and Disclosure. You may request, up to two times every 12 months, that we disclose the categories and specific pieces of Personal Information we have collected about you, the sources from which it was collected, the business or commercial purposes for collecting it, the categories of third parties with whom we shared it, and a portable copy of your Personal Information.
  • Correction. You may ask us to correct inaccurate Personal Information we hold about you. We may request documentation to validate the correction. If we determine the information is inaccurate, we will correct, delete, or identify the source of the information within 45 days. You may submit a written statement (up to 250 words) concerning alleged inaccuracies in health-related information, which will be made part of your record.
  • Deletion. You may request deletion of Personal Information you have provided to us, subject to certain exceptions allowed under applicable law (e.g., legal compliance, dispute resolution, contract enforcement).
  • Opt Out of Targeted Advertising. You may request that we stop using your Personal Information for targeted advertising, including use of cookies and trackers.   • Limit Use of Sensitive Personal Information. You may request that we limit our use of your Sensitive Personal Information (such as health information or government-issued ID numbers) to what is necessary to perform the services you have requested.
  • No Discrimination. We will not discriminate against you for exercising any of these rights. We may deny certain requests, in whole or in part, based on our legal rights and obligations.

These rights do not apply to Protected Health Information (PHI) governed by HIPAA. Please refer to our Notice of Privacy Practices for rights regarding your PHI.

How to Submit a Request

We may ask for identity verification prior to fulfilling your request. We will confirm receipt of your request within 14 business days and respond within 45 days. We may extend this period by an additional 45 days when reasonably necessary, with notice provided within the first 45-day period.

Authorized Agents

You may designate an authorized agent – a natural person or business entity that you have authorized to act on your behalf – to submit privacy requests for you. We may require a written power of attorney or signed permission confirming the agent's authority, as well as direct verification of your identity. Once verified, your authorized agent may act on your behalf.

Appeals

You may have the right to appeal a decision we make concerning your privacy request. To submit an appeal, contact us at hello@prostho-dent.com with the subject line “Privacy Request Appeal” and include the details of the original request and the basis for your appeal.


B. Email and SMS Communications

When you provide us with your email address or phone number as part of the registration or appointment-setting process, we may add you to a list to receive informational and promotional communications. Prostho Endo Dental Group may use third-party vendors to help manage these communications. While we supply vendors with your contact details, your information is never used for any purpose other than to communicate with you on our behalf.

Email: You can opt out of receiving marketing emails at any time by clicking the “unsubscribe” link in the email footer. Note that your opt-out applies only to the email address used.

SMS Text Messages: You can opt out of receiving SMS text messages at any time by replying STOP (or such keyword as directed within the message).

Email and SMS text messages are not fully secure communications. By providing your email address and/or mobile number, you consent to receiving communications that may contain Protected Health Information (PHI) in those communications. We never sell your email address, mobile number, or text messaging opt-in data or consent to third parties.

We may continue to contact you to the extent necessary for the purposes of any products or services you have requested and our ongoing business relationship.


C. Opt-Out Preference Signals & Global Privacy Control

You have the right to submit a request to opt out of the sale and sharing of Personal Information through an opt-out preference signal or Global Privacy Control (GPC). If you use such a signal, we will apply the opt-out to the device or browser sending the signal, as well as any consumer profile associated with that browser or device. You can enable a GPC signal by downloading a supported browser or extension.

8. Minor Children

This website is not directed to children under the age of 18. We do not knowingly collect or solicit Personal Information from minors under 18 years of age, and we request that such individuals do not provide Personal Information through our website. Parents and legal guardians are welcome to provide information on behalf of their minor children.
